•   Five Wishes: an easy-to-use legal document that lets adults of all ages plan how they want to be cared for in case they become seriously ill. DVD available as a helping tool when planning. Available in 20 languages.

•   Next Steps: a guide on discussing and coping with serious illness, is a companion to Five Wishes.  The guide includes information on completing Five Wishes, talking to family members about completing a living will, and discussing Five Wishes with your doctor. 

•   Family Resource Guide: provides a list of some of the best resources designed for family caregivers including advance care planning, caregiving, funeral planning, end-of-life care, grief and bereavement, hospice, long term care, pain management, and spiritual/psychological issues.
